Registration & Immunizations

At Coudersport ASD, we understand that you have high hopes and expectations of your child, your family, and us. We share that hope and expectation with you, and we are committed to making them a reality as we work together to prepare your child for the future.

Registration

To register your child at a Coudersport ASD, you will need to provide the following documentation, in person, at the office of the school your child will attend:

  • Child’s original birth certificate or a certified copy

  • Immunization records

  • Proof of residency—include two forms of documentation such as a deed/lease; utility, credit card, or property tax bill; driver’s license, or vehicle registration.

Kindergarten Registration
Children who turn five years old on or before September 1 of the entering school year may enroll in kindergarten.

Homelessness Program

The McKinney-Vento Homeless Assistance Act can help provide school stability even when you do not have a permanent home. We directly enroll all migrant/homeless students into our district even if parents/guardians cannot produce the required documentation. The district works with such students to track down personal records. We do not inquire about immigration status during the enrollment process.

Immunizations

The state of Pennsylvania requires that all students receive immunizations unless exempted for religious or medical reasons. Coudersport ASD adheres to this law to safeguard the school community from the spread of certain diseases. We do not admit students to school without proper immunization records and may remove unvaccinated students from school during an outbreak.

In addition to providing the required documentation, please plan to complete a registration packet (provided at the school of attendance) that includes the following: 

  • Registration forms

  • Parent statement attesting to whether the student has been or is suspended or expelled for offenses involving drugs, alcohol, weapons, or infliction of injury or violence on school property

  • Home language survey
